Office Fees, Insurance, and Gift Certificates

•Initial appointment is $135 which includes an intake and history

as well as a first NeurOPTIMAL™ Neurofeedback Brain Training Session

•Sessions thereafter are $100

Many clients come to do both brain training and counseling and coaching.  You determine what you want.

(there is a reduced rate available with my office partner and Certified NeurOPTIMAL Trainer)

CREDIT/Debit CARDS:

 

Payment needs to be made by the time of the session please. 

 

Please Note: *Debit Cards fee is 1.6% plus .25 per transaction. **Credit card fee will be added- 3%  plus .25 cent fee per transaction- as charged by the charge card Merchant provider. 

 

 

Economic Hardship

Boulder Neurofeedback recognizes that we all need help at times, regardless of how much we make.  As such, we offer a limited number of sliding scale slots at a discounted rate to serve those in the community who cannot afford our standard fee, and need support.  To see if you qualify and if space is available, please contact us today!

 

 

INSURANCE:

**My office will submit for Workman’s Compensation, OUT OF NETWORK INSURANCE and MED-PAY AUTO INSURANCE, for auto accidents.  *We are also a Denver Options Traumatic Brain Injury Service Provider(State of Colorado TBI Fund), the Colorado State of Colorado Department of Human Services Division of Vocational Rehabilitation (DVR), and will bill them directly for your approved services with authorization.

 

We are otherwise happy to provide you with a receipt so that you can seek all other insurance reimbursement; we will provide you with a diagnosis and treatment code.

 

GIFT CERTIFICATES:

Gift certificates are available in any amount.  Please contact me and I will arrange a certificate in the name of the person receiving the gift.
